Appointments and Consultations

Appointment requests submitted online are not confirmed until our office confirms them with you. Consultations are thorough clinical assessments with a provider and may carry a fee. Treatment recommendations are based on your individual evaluation, and not every treatment is right for every patient.

Deposits, cancellations, and no-shows. A $50 deposit is required to secure your appointment with Leah Michel, APRN, FNP-BC. The deposit is applied toward your service at the time of your visit. If the deposit has not been paid at least 24 hours before your scheduled appointment, your appointment will be cancelled.

If you reschedule with more than 24 hours' notice, your deposit will be transferred to your new appointment. If you cancel within 24 hours of your appointment, arrive more than 15 minutes late, or do not show up, your deposit will be forfeited.

Payment and Financing

Payment is due at the time of service unless other arrangements have been made. We accept the insurance plans listed on our website for eligible medical services. Cosmetic and aesthetic services are generally not covered by insurance and are the patient's financial responsibility.

Financing options offered through third-party providers are subject to that provider's own terms, approval, and policies.

Cosmetic Membership

Our Cosmetic Membership is governed by a separate membership agreement provided at enrollment. If there is a conflict between these Terms and the membership agreement, the membership agreement controls.

Promotions, Specials, Giveaways, and Events

Unless stated otherwise:

• Promotions and specials are valid only during the stated dates and while supplies last

• Offers have no cash value, cannot be exchanged for cash, and cannot be combined with other offers

• Offers may be limited to one per patient

• Treatments purchased through a promotion still require a provider evaluation to confirm you are a suitable candidate

• Giveaways and contests are subject to their official rules, which will be provided with each giveaway

• Event pricing, reservations, and deposits are subject to the terms shared at booking

We may change or end any promotion at any time as permitted by law.

Results Disclaimer

Results of medical and cosmetic treatments vary from person to person. Before-and-after photos, testimonials, and reviews on our website and social media reflect individual experiences and are not a guarantee of results. Your provider will discuss expected outcomes, risks, and alternatives with you before treatment.

Skin Care Products

Product descriptions and prices are subject to change. Use products as directed and stop using any product that causes irritation or an adverse reaction, and contact our office.
